High-Frequency Oscillatory Ventilation (HFOV) is a rescue mode for severe ARDS that uses supraphysiologic frequencies and low tidal volumes to maintain a constant mean airway pressure for oxygenation, but its complex physiology makes it challenging to manage. Despite early promise in pediatrics, major adult trials (OSCILLATE and OSCAR) demonstrated either increased mortality or no benefit over conventional ventilation, leading to its decline as a routine strategy. Current evidence supports using HFOV only as a last-resort, expert-guided therapy in carefully selected patients, with strict attention to hemodynamics and contraindications like obstructive lung disease.
